In an effort to better serve its Aetna Medicare Advantage Members, Aetna is joining forces with MNS to reach out to physicians and certified registered nurse practitioners who care for Aetna Medicare Advantage Members, and asking that they complete an annual assessment and care plan for Members who have been admitted to a skilled nursing or long-term care facility. This program has been designed to improve quality of care and help reduce avoidable hospital admissions and readmissions.
